Furthermore, applicant agrees KVC Logistics can check all references provided, as well as any others that that become available to KVC Logistics during their credit review process

Applicant agrees to the need for verification of all information on this application and authorizes and releases the banks, businesses, and persons identified on this application to furnish any and all information requested by KVC Logistics Corp its agents or representatives, by telephone or written correspondence, whichever KVC Logistics requests.
13. Applicant understands KVC Logistics requires motor carriers under contract with them to maintain cargo loss and damage liability insurance in the amount of $100,000 per shipment. By signing below, applicant acknowledges that loads valued in excess of $100,000 will not be tendered by applicant without first notifying KVC Logistics in writing of the actual value of the shipment. This is necessary to allow KVC Logistics the opportunity to arrange for the additional insurance prior to shipment. Failure to provide timely written notice will result in your loads not being insured to the extent that the value of the shipment exceeds $100,000.
